Fees are refundable for Pre-pay students only; Weekly-pay students will only be given class credit ($15 per class***). A 72-hour notice must be given in writing or via email before the class start date; then a prorated fee ($15 per class***) is returned on classes not taken.

Otherwise, the student can purchase a Personal instruction time slot ($20/hour) with the teacher to go over the missed lesson if available.  

    • Pre-pay Semester students -  can purchase a Personal instruction time slot ($20/hour) with the teacher to go over the missed lesson if available.  

    • Pre-paid Total Program students -  can schedule a Personal instruction time slot with the teacher to go over the missed lesson if available. (Max: 1 hour for free per missed class; otherwise $10/hour)   

School Closures

Should the school have to cancel classes for any reason, students will be notified via their preferred contact route/point within 1 hour of determination. Refunds will not be given, but the missed class will be taught the following class period, extending the semester for the additional day/s missed.

WE HAVE MISSED YOU !! Re-enrolling

Any student can re-enroll exactly where they left off in the program for the next class start date, enrolling only as a Pro-rated Pre-pay student at the rate of $20 per class. 

Injury/Sickness Claim Waiver

Please read this section carefully and be aware that in consideration for JA*** to provide educational services it may require the use of dangerous machinery with moving parts. You will be expressly assuming the risk and/or legal liability and waiving/releasing all claims for injuries, damages, or loss that you or your enrolled minor child/ward might sustain as a result of said services.

Please read this section carefully and be aware that in consideration for JA*** to provide educational services it may require the use of dangerous machinery with moving parts, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which your minor child/ward might sustain as a result of said services.

I recognize and acknowledge that there are certain risks of physical injury to handling Sewing Machinery, and I voluntarily agree to assume the full risk of any injuries, damages, or loss, regardless of severity that myself and/or my minor child/ward may sustain as a result of participating in any and all activities connected with or associated with JA*** Programs, including but not limited to, injuries, damages, and loss arising out of any activities on JA***.  I further agree to waive and relinquish all claims myself and/or my minor child/ward may have (or accrue) against JA***, including its respective officials, agents, and employees.

I do hereby fully release and forever discharge the Parties from any and all claims for injuries, damages, or loss that myself and/or my minor child/ward may have, or which may accrue or arise out of, connected with, or in any way associated with said JA*** Programs.

Adult Student:

I give permission for JA*** owners, officers, teachers, employees, and/or agents to seek emergency medical treatment for myself in the event I am not capable of assisting myself or deciding my own medical treatment.

 Minor Student:

I give permission for JA*** owners, officers, teachers, employees, and/or agents to seek emergency medical treatment for the minor participant(s) in the event they are unable to reach any Parent or Guardian.

The undersigned (see registration form) also agrees that they, themselves will be responsible for any financial debt incurred due to any injuries, sickness, or loss.

I further agree that this agreement shall be governed by the laws of the Commonwealth of Kentucky.

I have read and fully understand the above waiver and release of all claims.

Promotional Video/Picture Photography Waiver

Occasionally JA*** uses photos or videos of its students in print ads, on its websites, or in other marketing mediums. I understand that my child's likeness may be used in such advertising. These images will be used for JA*** purposes only and will not be given or sold to outside companies or individuals. *If I do not agree with this policy, I must submit a statement in writing to JA***.
